All participants undertaking any scuba diving activity are required to complete a self-assessment medical declaration upon registration at the dive centre.

If the participant is deemed safe to dive no further documentation is required.

On the other hand if the medical assessment highlights any risk factors the dive centre is required by local law to request clearance from a qualified medical physician.

It is therefore recommended to request a copy of the medical questionnaire well in advance prior to the date you plan to dive so this can reviewed and if necessary issue the required medical clearance in time.
